If you're an individual looking to order State/FBI cards please contact your local state agency/department/board as they will have the cards to give you. If you are an agency/department/board that the DCI houses your approved FBI cards please continue with filling out and submitting the following form to order fingerprint cards from the Division of Criminal Investigation.
